Returning Starting School Questionnaires (DN, PS and CM only)
Please remember that the Starting School Questionnaires are due to be returned to the Healthy Child Programme (HCP) by Friday 1 November.
Before returning the questionnaire, please review the contents, add any new information to your CLaWBA, and indicate on the form if you are able to deal with issues raised or if they will need to be addressed with families by HCP.
Please return the reviewed questionnaires by either scanning them and emailing to your local Healthy Child Programme team or dropping them into your local Children and Family Centre marked for the attention of the Healthy Child Programme.

You can still return any completed questionnaires that are returned after 1 November, but please return as soon as possible afterwards so support can be provided to families at the earliest opportunity.
