Dear partner,
This section has been designed to support RPs with identifying local services for vulnerable residents and for facilitating referrals.
This content helps to implement the aims of the 'Neighbourhood and Community Safety' and 'Health and Resilience in Housing' Work Plans.
'Partnership problem-solving' meetings and 'Joint Weapon Sweeps'
RPs can attend Partnership problem-solving meetings and Joint Weapon Sweeps can also be arranged by contacting rob.harper@lambeth.gov.uk
Ward Panel Meetings – Winter 2024/2025
Ward panels are local meetings run by the Metropolitan Police. At a ward panel meeting, residents are encouraged to engage with their local policing team, discuss local crime and disorder concerns and ultimately inform the promises and priorities of their local Safer Neighbourhood teams.
See below link to all Lambeth’s Safer Neighbourhood Teams and details of upcoming meetings and events and the named officers within Lambeth’s 25 wards. In addition, details of local policing priorities and crime maps can also be found here too.

Referrals - Frequently Asked Questons (FAQs)
FAQ: How can I refer a Lambeth resident to mental health services?
-
The Single Point of Access (SPA) helps adults aged 18 to 65 get the right mental health support. RPs may can contact the SPA if they are worried about someone’ mental health and if they need more support than a GP can provide. Self-referrals can also be made here too.
To refer to this service:
-
Call them on 0800 090 2456 (Monday to Friday 9am to 5pm)
-
After they receive your referral, they will work to find the right help. They may need to call the person to discuss this and agree a plan.
-
Contact mental health services on behalf of the person so they get help quickly, or
-
Inform the person about community services so they can contact them themselves
FAQ: Can anyone be referred to this service?
-
Any Lambeth resident between the ages of 18 – 65 years who are seeking support with their mental health can be referred.
However, the SPA does not currently provide a service to:
-
People from outside of Lambeth
-
Where the person does not have a mental health problem
-
Where there is a primary diagnosis of substance misuse, without a co-existing mental health problem
-
Those with neurodiversity in the absence of a co-existing mental health problem or;
-
Those with organic disorders such as a decline in mental function due to a medical or physical disease.
FAQ: What if there are safeguarding concerns about any abuse or risk of abuse for a resident?
- Any safeguarding concerns must be escalated immediately.
- If you need to raise an adult safeguarding concern about a resident experiencing abuse or neglect and who cannot protect themselves, report it using the online form - this is the quickest and most secure way to report concerns. The person telling us about the possible abuse or neglect can remain anonymous.
-
New Cases and existing cases are referred to Lambeth Safeguarding HUB.
FAQ: How do I refer a resident for an Adult Social Care needs assessment and/or an Occupational Therapy (OT) assessment?
-
When a professional (or member of the public) are concerned about the welfare and wellbeing of a resident. This might be for a general referral to Adult Social Care (ASC) for a care needs assessment and or a Occupational Therapy (OT) assessment)
- Please ensure to discuss the referral with the resident to obtain their consent.
Referral process to Lambeth Adult Social Care (ASC)
The Nature of ASC referrals:
-
Existing cases - People who are already in receipt of ASC services (Care package, Day services & in placement). These existing cases are referred to Community Duty Team.
-
New cases - People who are not in receipt of ASC services (could still be on ASC systems with a minor equipment in place like a commode, bath lift). New Cases are referred to Age UK Lambeth or iCAS (information, Contact & Assessment Service).
FAQ: How do I report any environmental health concerns related to hoarding and/ or pest control issues?
-
Lambeth Council’s Self-Neglect and Hoarding Guidance (2017) is being reviewed by Lambeth Safeguarding Adults Board (LSAB). It provides practical guidance for dealing with self-neglecting or hoarding residents. It is essential to build trust first and to engage with compassion to understand individual circumstances and emotional states, so that informed and effective interventions can be made.

-
Cost of living, money and debt. RPs can refer affected residents to Lambeth Council’s Community solutions pages, which list various offerings to help address these challenges.
The 'Housing and Health Network’
The LHP works closely with the network, which seeks to improve the links between health and housing to improve support for Lambeth residents.
RPs are invited to attend the monthly ‘Housing and Health Network’ meetings, which are organised by ruth.vidal-tunkara@nhs.net
The network's main aims are to:
- improve understanding of the remit, limitations and challenges of each other's roles
- build better relationships
- improve information sharing and create pathways to escalate cases when necessary
- identify opportunities for reciprocal training
Voluntary and Community Sector (VCS) and Social Prescribing
It is important for RPs to make preventative outreach and maintain updated local contact information for local community groups to consider holistic options from the Voluntary and Community Sector (VCS) and making use of social prescribing supports.
Lambeth's Voluntary and Community Sector (VCS) play a vital role in the lives of people who live in the borough. It has a strong track record in developing and delivering a range of innovative support services. To see a list of Lambeth's voluntary, community, and social enterprise organisations, visit the Community Lambeth directory. National charities such as the Samaritans and Hoarding UK, can also assist tenants in accessing emotional and practical help.
Age UK were commissioned to run the MYcommunity Directory and individual organisations keep their listings updated.
Social Prescribing is a way of connecting people to non-medical support within their community to improve their health and wellbeing. Social prescribing became part of the NHS long-term plan to provide nonclinical support to patients, as 1-5 patient visits are repeats and not related to medical needs. In Lambeth, there are 9 Primary Care Networks employing eleven link workers.
RPs can refer individuals through Lambeth social prescribers to assist those with long-term conditions, mental health issues and complex social needs. The service takes a holistic approach by addressing the social determinants of health that influence an individual’s wellbeing. Link workers connect clients to non-clinical support services provided by local charities and groups. e.g. MYcommunity Directory and Community Lambeth directory.
The LHP hosted a dedicated 'Social Prescribing Awareness Session' (19.9.24) for RPs on when to use social prescribing interventions and how refer to link workers. This is a holistic approaches to patient care, which addresses underlying social issues like loneliness and uses both formal and informal referral processes.
