Upcoming Health Education Program Opportunities

 

Dear Montana Partners,

 

The Montana Diabetes Program (MDP) is excited to offer two new health education programs to Montana communities. MDP is looking for organizations interested in implementing the new DECIDE Diabetes Problem-Solving Support Program (DSP) or the new MEND Healthy Together Family Health Weight Program (FHWP). 

Each participating site is eligible for up to $5,000.00 to support program activities. The MDP also provides training, program curriculum, program materials, implementation support, outreach materials, and more.

The Montana Diabetes Program will host an informational meeting on 4/23/2025 at 12:00pm for all who are interested. Meeting information is provided below.
