Update on the Youth Homelessness Coordinated Community Plan
Tracking progress – new progress report
To track the advancement of the CCP, the Youth Action Board worked with Hennepin County to develop the Coordinated Community Plan – Progress Report. The Progress Report includes status updates for all goals, objectives, and action steps outlined in the Coordinated Community Plan. This will guide our community’s ongoing and future work to prevent and end youth homelessness.
Get involved
There are two ways that you can get involved:

Submit a survey! Review the Coordinated Community Plan – Progress Report and identify goals that you or your program may have impacted or influenced. If you have an update, submit a survey.

Attend an update session! Attend the Coordinated Community Plan – Progress Report update session with Collaborative Solutions and Homebase, technical assistance groups for the Youth Homelessness Demonstration Program (YHDP).
Hennepin County CoC funding committee is seeking new members
The HUD Continuum of Care (CoC) funding committee is seeking new members. As a committee member, you will review data and make recommendations to the Hennepin CoC Governing Board. We encourage applicants with experience navigating social service systems and homelessness to apply. Meetings are held virtually via Microsoft Teams on the 2nd Wednesday of each month from 1:00-3:00 PM.
Essential job responsibilities:
- Participate in one two-hour meeting per month
- Review materials before meetings as needed
- Use personal experiences to provide insights on funding decisions
- Listen to and consider other perspectives
- Complete an application to apply

Interested candidates should complete and submit the Hennepin County funding committee application.
The application deadline is February 7, 2025. It can be completed by phone or submitted via email.

The Request for Proposals (RFP) for Community Development Block Grant (CDBG) funds is now open to applications. An estimated $400,000 is available for social services, an estimated $1,100,000 is available for funding capital and community development projects, and an estimated $230,000 for fair housing activities:
- Social services
- Capital and community development
- Fair housing
All CDBG activities must meet national objectives and meet a priority need of the Five-Year Consolidated Plan for suburban Hennepin County.
