A Provider Scorecard was added to the CES Provider Level Dashboard.
The CES Evaluation workgroup reviewed standard deviation and interquartile data to establish thresholds used to quickly look at four metrics related to CES referrals:
- Average Days from Referral to Result
- % of Referral Result within 1 month
- % Successful Referral
- Intake to Housed
The scorecard gives housing providers a quick snapshot of how their programs are performing compared to the thresholds established (below expectations, meeting expectations, or exceeding).
The scorecard is used to highlight providers exceeding expectations and offer technical assistance to programs struggling to meet expectations.
The dashboard is not public facing but any housing provider accepting referrals through CES can have access to the dashboard.

Prioritization based on Medical Fragility rolled out in the Singles system on August 1st, 2023.
Now we want your feedback on the medical fragility questions.
Specifically, we want to know any assessments where you believe the answers the client gave to the medical fragility questions did not reflect their actual level of fragility.
Let us know here if you believe there is a discrepancy between how the medical fragility questions were answered by the client and their actual circumstances, thus negatively impacting their prioritization for a housing referral.
After we collect your responses we will bring them to the Lived Experience Advisory Group for further discussion.
