Hennepin County’s CoC partnered with C4 Innovations to evaluate the strengths and challenges of its Coordinated Entry System to further strengthen their model and understand how to best support future enhancements within the CoC.
These recommendations will further strengthen the homeless response system in the CoC’s goal to end homelessness.

As part of HMIS workflow housing providers update the referral result in Coordinated Entry Event.
There are 3 options from which to choose:
- Successful Referral: Client Accepted
- Unsuccessful Referral: Client Rejected
- Unsuccessful Referral: Provider Rejected
When the referral is unsuccessful, providers select a reason why.
Moving forward, please use the following guidance when selecting the reason the referral was unsuccessful:
Unsuccessful Referral: Client Rejected – Always Select “Client refused services.”
This is the ONLY response that should be selected with “Unsuccessful Referral: Client Rejected.”
Unsuccessful Referral: Provider Rejected – Select the most accurate option. For example: Client Unreachable, Client is Not Eligible, etc.
In the very near future clients will NOT get a referral unless the Medical Fragility questions are answered completely.

Then, contact the clients and update the assessment with their answers.
