The Early Intensive Developmental and Behavioral Intervention (EIDBI) provisional license application is now live in the Provider Hub!
Providers enrolled with Minnesota Health Care Programs (MHCP) as EIDBI agencies can create their Provider Hub account and begin the application.
Review this message for help getting started in the Provider Hub and reminders on required actions/timelines.
The individual who will serve as the Authorized Agent for the EIDBI agency should create a Provider Hub account. They’ll use their account to access the provisional license application in the Provider Hub.
What is an Authorized Agent?
The EIDBI agency must designate one of its controlling individuals to be the Authorized Agent (as defined in Minnesota Statutes, section 245A.02, subdivision 3b). The Authorized Agent is the person responsible for communicating with Minnesota Department of Human Services (DHS) Licensing.

Before the Authorized Agent starts the EIDBI provisional license application in the Provider Hub, it may be helpful to do these things first:
Review the Pre-Licensing Application Checklist (PDF). This checklist can help you prepare to complete your provisional license application. The checklist is not required; it is not the application for provisional licensure. It was originally shared with enrolled EIDBI agencies in December.
Gather the following information:

Enrollment record ID — Find this in the Minnesota Provider Screening and Enrollment (MPSE) portal by logging in to your MN-ITS account.
- Federal Employee Identification Number (FEIN)

Provider identifier — Either a National Provider Identifier (NPI) or Unique Minnesota Provider Identifier (UMPI). Find this in the MPSE portal or the Welcome Letter sent by the Minnesota Department of Human Services (DHS) when your enrollment was initially approved by Minnesota Health Care Programs (MHCP).
You’ll need this information to “claim” (or access) your provisional license application in the Provider Hub. Only enrolled EIDBI agencies are allowed to complete the application.

Authorized Agent: Complete the Applicant Notarized Agreement, Acknowledgement and Verification Form (PDF) in the presence of a notary public. The Authorized Agent will need to upload the signed and notarized form in the Provider Hub as part of their application.

When ready, the Authorized Agent should log in to the Provider to start the EIDBI provisional license application.

If you need help with the online tool, submit a case when you're logged in to the Provider Hub or call 651-431-6075. The Product Success team responds to cases Monday through Friday between 8 a.m. and 4:30 p.m.
EIDBI provisional license application(s) are due in the Provider Hub by May 31, 2026.
EIDBI agencies that have not submitted an application by this date must not operate and will be disenrolled from providing EIDBI services.
The Provider Hub is an online tool to apply for and manage social service licenses and participation in available government programs.

Attend an information session — topics may include recent improvements to the Provider Hub, tips for using the online tool effectively and new help materials. Attendees are welcome to submit questions during the session. The Provider Hub team hosts a public, online session about every two months. The next session is tentatively scheduled for March; the exact date will be announced in the February newsletter. Find recordings from previous information sessions on the Provider Hub webpage.
The Minnesota Legislature is requiring all enrolled Early Intensive Developmental and Behavioral Intervention (EIDBI) agencies to have a provisional license.
