In January 2026, a provisional license application will launch to providers enrolled with Minnesota Health Care Programs (MHCP) as Early Intensive Developmental and Behavioral Intervention (EIDBI) agencies.

EIDBI agencies will complete this provisional license application in an online tool called the Provider Hub. This message shares introductory information about the Provider Hub.

Do not create a Provider Hub account yet. You will receive another email when the tool is ready for you to use.


About the Provider Hub

The Provider Hub is an online tool to apply for and manage social service licenses and participation in available government programs.

What EIDBI agencies will do in the online tool

When the EIDBI provisional licensing application launches in the Provider Hub, enrolled EIDBI agencies will use it to fill out and submit their provisional license applications, pay license application fees, view approved licenses and get help.

  • Use the online tool in English, español, Soomaali or Hmoob
  • Get technical help both from online articles and a local support team

  • Subscribe to the Provider Hub Newsletter to get updates on new and improved features in the Provider Hub, resources, upcoming events and other news about the online tool. The Provider Hub team sends a newsletter about every two months. Check out the latest newsletter sent Dec. 12.
  • Attend an information session — topics may include recent improvements to the Provider Hub, tips for using the online tool effectively and new help materials. Attendees are welcome to submit questions during the session. The Provider Hub team hosts a public, online session about every two months. Join us for the next one Tuesday, Jan. 20 from 6 to 7 p.m.

  • Review the Technology Resources Learning Library for help building comfort with the technology you’ll need to use the Provider Hub. You can take a technology skills self-assessment and explore a tech skills guide. Each resource is available in English, español, Soomaali and Hmoob.

Actions and Dates

Before you start your application

Before starting the provisional license application in the Provider Hub:

Review the Pre-Licensing Application Checklist (PDF). This checklist can help you prepare to complete your provisional license application once it’s ready in the online tool. The checklist is not required; it is not the application for provisional licensure. It was originally shared with enrolled EIDBI agencies in December.

Gather the following information:

  • Enrollment record ID — Find this in the Minnesota Provider Screening and Enrollment (MPSE) portal by logging in to your MN-ITS account.

  • Federal Employee Identification Number (FEIN)
  • Provider identifier — Either a National Provider Identifier (NPI) or Unique Minnesota Provider Identifier (UMPI). Find this in the MPSE portal or the Welcome Letter sent by the Minnesota Department of Human Services (DHS) when your enrollment was initially approved by Minnesota Health Care Programs (MHCP).

You’ll need this information to “claim” (or access) your provisional license application in the Provider Hub once it’s ready for you. Only enrolled EIDBI agencies will be allowed to complete the application.

Determine your Authorized Agent. The EIDBI agency must designate one of its controlling individuals to be the Authorized Agent (as defined in Minnesota Statutes, section 245A.02, subdivision 3b). The Authorized Agent is the person responsible for communicating with the Minnesota Department of Human Services (DHS) Licensing. Find more information about this role on page two of the Pre-Licensing Application Checklist (PDF).  

Authorized Agent: Complete the Applicant Notarized Agreement, Acknowledgement and Verification Form (PDF) in the presence of a notary public. The Authorized Agent will need to upload the signed and notarized form in the Provider Hub as part of their application.

After application launches — January 2026 

Later this month, the State of Minnesota will send an email announcing the provisional license application is ready in the Provider Hub. This message will be sent to the email address associated with the enrolled EIDBI agency.

After the agency receives this message, the individual whom the agency has identified as the Authorized Agent should create a Provider Hub account and begin the application. The email from the State of Minnesota will include instructions on how to do this. Do not create a Provider Hub account until you receive this message.

May 31, 2026

Provisional license application(s) are due in the Provider Hub by May 31, 2026. EIDBI agencies that have not submitted an application by this date must not operate and will be disenrolled from providing EIDBI services.


About EIDBI Provisional Licensing

The Minnesota Legislature is requiring all enrolled Early Intensive Developmental and Behavioral Intervention (EIDBI) agencies to have a provisional license.
