Updates to the Alternative Care Program and Essential Community Supports Eligibility Worksheets

Updates to AC Program and ECS Eligibility Worksheets

DHS Aging and Adult Services Division (AASD) updated the Alternative Care (AC) Program and Essential Community Supports (ECS) Eligibility Worksheets because of annual updates to the new Federal Poverty Guideline (FPG) limits, rates and income standards.

AASD also updated the projected nursing facility care cost on the worksheets. We sent an eList announcement in January 2022 that explained why we were unable to update this amount. AC Eligibility Worksheet changes effective July 1, 2022

We updated AC Program Eligibility Worksheet, DHS-2630 (PDF) and AC Program Eligibility Worksheet Type A, DHS-2630A (PDF) with changes to the following areas:

  • Changed the form title of DHS-2630 to “Alternative Care Program Eligibility Worksheet”
  • Personal Needs Amount
  • Projected Nursing Facility Care cost for 135 days + MA asset limit amount
  • Statewide Average Payment for Skilled Nursing Facility (SAPSNF) care rate
  • FPG amounts for proper eligibility determination
  • Income standard for QI-1/QMB/SLMB lead agency referral amount
  • Spousal Allocation amount on DHS-2630A
  • Community Spouse Asset Allowance amount on DHS-2630A

ECS Eligibility Worksheet changes effective July 1, 2022

We updated ECS Financial Eligibility Worksheet, DHS-6683 and ECS Eligibility Worksheet for a Married Individual when only one spouse is requesting services, DHS-6683A with changes to the following fields:

  • Personal Needs Amount
  • Projected Nursing Facility Care cost for 135 days + MA asset limit amount
  • FPG amounts for proper eligibility determination
  • Income standard for QI-1/QMB/SLMB lead agency referral amount
  • Spousal Allocation amount on DHS-6683A
