Form updates
DHS has updated the following documents:
Use the updated version, which
has been available since Aug. 15, 2018.
Changes made:

Removed the value 00 for the (SNBC) emotional field. Use
value 12 instead.

UMPI Number field in Section A now accepts
all ten digits of the UMPI number

The LTCC field in Section A
now accepts value HHP for Hennepin Health Plan
- The Emotional
Health question no longer has value 00 as an option. Use value 12
instead.
- The Subjective Evaluation of Health section was reduced
- The Medication List field was increased to allow more room to answer
- The Hospital/Nursing
Home questions now allows zero as a value when the member was not present
in these settings (Note: The value will transfer to form DHS-3427H as a
zero)
New forms
Use this new form for Activity

Updated instructions
The updated August 2018 version
replaces the previous version in eDocs. See Page 10 of the document for a list of changes.
DHS updated the version Aug.
6, 2018, to correct value names used for behavioral/emotional
fields.
