Reminder: Respond to case management redesign survey before Nov. 4

PURPOSE: Reminder to respond to the case management redesign survey by Friday, Nov. 4, 2016

On Oct. 7, 2016, DHS sent a memo to county/tribal partners and stakeholders that asked them to review and react to a series of documents developed to help inform the next phase of planning for case management redesign. 

These documents, referred to as "set-up documents" are on the DHS case management redesign site.  We ask that you review any or all of these documents and take the case management redesign set-up survey to provide your feedback.  Survey responses are due by the end of the day, Friday, Nov. 4, 2016. 

We thank you for your partnership and support as we continue to work toward a unified vision for case management redesign.
