Dear Skilled Nursing Facilities in Los Angeles County,
The Los Angeles County Department of Public Health (LAC DPH) is officially accepting applications for the 2023-2024 Updated COVID-19 Vaccine Financial Reward for Skilled Nursing Facilities as of today Monday, November 20, 2023. The link to apply online is here. This opportunity is available to the first 100 skilled nursing facilities (SNFs) to apply from Los Angeles County, including Pasadena and Long Beach, or until funds run out. Please see below for reward requirements. If a facility qualifies for both rewards, they could be rewarded $10,000 in total. The deadline to submit is Friday, December 22, 2023.
$7,500 Financial Reward
- Facilities must demonstrate that 80% or more of their resident census is up to date with the 2023-2024 formulation of the COVID-19 vaccine. The resident census must include everyone who resided at the facility at any time over a 1-week period from Monday through Sunday between Nov 13 and Dec 17, 2023.
AND
- Facilities must also demonstrate they’re reporting all administered vaccine doses to the California Immunization Registry (CAIR2).
$2,500 Financial Reward
- Facilities must demonstrate 80% or more of their staff census is up to date with the 2023-2024 formulation of the COVID-19 vaccine. The staff census must include everyone who worked at the facility at any time over a 1-week period from Monday through Sunday between Nov 13 and Dec 17, 2023.
Supporting documentation is required. All facilities must submit the LAC DPH SNF COVID-19 Financial Reward Application and upload person-level COVID-19 vaccination data in the application portal, which is secure. Attached is a job aid on submitting the required supporting documentation briefly summarized below:
Resident person-level COVID-19 vaccination data can be provided 1 of 2 ways:
- Using the LAC DPH template: LACDPH_EventLevelCovid19VaccLtcRes.xlsx (linked here).
- Using NHSN: Select “Person-Level COVID-19 Vaccination Form- Residents” directly from the LTC component of the NHSN application COVID-19/Respiratory Pathogens Module under COVID-19/Respiratory Pathogens
Healthcare Personnel (HCP) person-level COVID-19 vaccination data can be provided 1 of 2 ways:
- Using the LAC DPH template: LACDPH_EventLevelCovid19VaccLtcHcp.xlsx (linked here).
- Using NHSN: Select “Person-Level COVID-19 Vaccination Form- HCP” directly from the LTC component of the NHSN application under COVID-19/Respiratory Pathogens
