Projects available include:
- Asthma
- Blood pressure / Hypertension (CMS 165)
- Cancer (breast, cervical, colorectal) (CMS 125, CMS 124, CMS 130)
- Cholesterol / dyslipidemia (CMS 347)
- Chronic Kidney Disease (CMS 134)
- Diabetes (CMS 122)
- Tobacco screening and cessation intervention/referral (CMS 138)
- Pre-diabetes
- Referrals to social or community programs for people with chronic disease

Hospitals, clinics, pharmacies, nursing homes, and more are welcome to apply.
Projects last one year and typically require monthly check in calls and some data tracking. In addition to funding, we'll provide technical assistance, a project manager, and free staff and/or patient education (project dependent).
