Word and PDF Versions Posted

DVR is pleased to share that the 2026-2028 Reports/Forms are now available as of June 29. This update includes revised and new report templates.

To support provider needs, both Microsoft Word and PDF versions of these forms are available.

In general, report templates were updated to:

• Align more clearly with the Technical Specifications
• Improve structure, readability, and consistency
• Support updated accessibility requirements for forms

Due to federal accessibility requirements, revised report templates have undergone structural and formatting changes to support compliance and usability.

Temporary Use of Word Versions

The availability of Word versions is being provided on a temporary basis while DVR continues working toward a fully accessible long-term solution.

Due to the extension of the federal accessibility compliance deadline to April 2027, DVR is able to continue offering Word-based forms during this interim period while we evaluate and develop alternatives that better meet accessibility standards, provider needs, and program requirements.

DVR remains committed to identifying and implementing a sustainable, accessible solution for required service documentation. This includes continued collaboration with state partners, providers, and other stakeholders to explore options that support both accessibility requirements and practical usability.

Next Steps

DVR will continue working toward long-term improvements for Reports/Forms and will share updates with providers as new solutions are identified and implemented.
