I am excited to see what we can do for the betterment of nursing and nursing practice. In my last column, I referred to recruitment and retention. I will continue to beat this drum as long as a forum is provided. Recruitment starts with retention. If we can retain nursing staff, does that lead to better recruitment? You bet it does! A happy nurse shares that news with others and talks about their work in a good light. A happy nurse tells younger people about the nursing field and how much change one nurse can make in a patient’s life. Having a positive work culture leads to so many rewards. Our occupation is facing an immense shortage of new nurses and those leaving the field. We can all help by working toward cultures of diversity, equity, and inclusiveness. We can all help by spreading the word about how rewarding this career is. Be part of the change.
As most of us went through nursing school, we learned about the Standards of Practice for Registered Nurses and Licensed Practical Nurses, or N6. This knowledge is valuable and helps build the foundation of our practice. However, we actually have seven chapters in our practice act. Today, I want to focus on a chapter few people are aware of, The Rules of Conduct or N7, for both LPNs and RNs. I would highly advise a review of this chapter as some of the rules may not be something that one would expect to have consequences. As the Board of Nursing (BON), we are tasked with reviewing the complaints submitted by members of the public or other agencies. A frequent violation is self-reporting of a conviction of either a misdemeanor or felony within 48 hours. When responding to the Board, nurses frequently will say they did not know they had to report it. Knowing N6 is needed for your practice. But it is your responsibility as a nurse to know the entire practice act, including the Rules of Conduct/N7.
N 7.03 Subsection H
Failing to notify the board of a felony or misdemeanor in writing within 48 hours after the entry of the judgment of conviction, including the date, place, and nature of the conviction or finding. Notice shall include a copy of the judgment of conviction and a copy of the complaint or other information which describes the nature of the crime in order that the board may determine whether the circumstances of the crime of which the credential holder was convicted are substantially related to the practice of nursing.
Finally, I have two words for all of you. Thank You for everything you do, day-in and day-out, no matter if you are at the bedside, desk, or classroom. We all contribute to this amazing profession in our own way.

Where Are Our Public Health Nurse Leaders?

Public health nursing is foundational to all nursing practice. Dating back to the late 1800s, public health nurses were instrumental in addressing social determinants of health and preventive care in community settings. In most states, policy and environmental changes have contributed to a diminishing public health nursing workforce over the past decade. Years of disinvestment in governmental public health created an underfunded and understaffed public health workforce that was greatly challenged by the COVID-19 pandemic (Wilson et al., 2020). Furthermore, the pandemic highlighted the need for health leaders that build and maintain the interconnection between medicine and communities (Bekemeier et al., 2021; Brownson et al., 2020). Historically, nurses have served in leadership roles across the governmental public health system, from local to state health departments. These Public Health Nurse (PHN) leaders have ensured the application of nursing practice and public health core principles with communities and other professions to address complex health issues (Bekemeier et al., 2021). Today, Wisconsin nurses are underrepresented in public health leadership. The Department of Health Services, Division of Public Health has a single nurse in a leadership position. Restoring nursing leaders is critical to an effective, comprehensive public health system.
A well-prepared public health nurse workforce is needed to ensure the health and wellness of the public. PHN leaders activate equitable policies and solutions to health problems for populations by applying principles of social justice to eliminate health disparities. They are effectively functioning as a bridge between clinical care systems and our communities. Some states require nurses to be certified to work in public health to ensure a well-prepared and qualified PHN workforce. To meet this requirement, a nurse can provide evidence of coursework in public health nursing through a certificate program or an advanced degree option such as a Master’s in Public Health (MPH) or Doctor of Nursing Practice (DNP) in Population or Public Health. Wisconsin does not have such a requirement.
Wisconsin has 86,996 registered nurses that renewed their license in 2022. Of these, 2,568 nurses are working in public health positions and 1,350 are working in governmental agencies. There are three ways Wisconsin can strengthen and restore the public health nursing workforce to encourage local health departments to hire RNs and position the state to better support communities to be healthy and respond to future public health crises. First, the Chief Health Nursing Officer at the Department of Health Services should be restored to provide a solid structure to ensure public health nurses practice to their full scope and adhere to the American Nurses Association standards. Second, a state certification process can be implemented and attached to the Wisconsin Nursing License. Last, nursing schools can restore public health curricula and experiential learning at the baccalaureate level and establish pathways to advanced practice nursing in the field of public health.

Stress impacts every facet of our lives and has become even more prevalent since the COVID-19 global pandemic. The pandemic profoundly impacted the lives of individuals by further exacerbating existing stressors and significantly limiting coping resources, resulting in a mental health crisis that could yield serious consequences for years to come. While all individuals continue to grapple with stressors in their daily lives, certain occupations are inherently stressful, further compounding the degree of stress that individuals experience. Nursing is widely regarded as one such profession.
Stress is one of the most underappreciated yet consequential issues that nurses face. Stress occurs when we are presented with physical, emotional, social, ethical, or moral stressors that exceed our available resources, resulting in an inability to adequately cope. While not all stress is bad, long-term stress can have detrimental consequences for individuals and organizations. On an individual level, stress can result in numerous negative health outcomes that can manifest as acute or chronic physical and/or psychological symptoms. Stress can lead to maladaptive coping mechanisms such as substance use, avoidance, and antisocial behaviors, which further negatively impacts relationships and individual wellness. Within healthcare organizations, stress can result in higher rates of absenteeism, turnover, burnout, patient care errors, work inefficiency, professional dissatisfaction, and poor quality of healthcare delivery (Milliken et al., 2007; Sarafis et al., 2016; Ticharwa et al., 2019). With turnover and vacancy rates on the rise, there is notable concern for how this will continue to compound the national nursing shortage. These concerning trends in nursing highlight the urgent need to proactively identify and address the factors that contribute to stress.
To mitigate the negative effects of stress and improve clinician wellbeing, various individual and organizational evidence-based stress management interventions and coping strategies should be considered. Below are some steps that individual clinicians can take to improve their overall wellness.

Recognize what stress and anxiety may look or feel like.
- Because people handle stress differently, stress symptoms can also vary (i.e., emotional symptoms, physical symptoms, cognitive symptoms, and behavioral symptoms).

Identify your stressors.
- We are often not fully aware of what causes us stress. Take some intentional time to check in with yourself. Learning how to recognize the sources of acute and chronic stress in your life is one of the first steps to take.

Cultivate healthy coping strategies.
- Take a break from news and social media.
- Take care of your body. Nurture yourself with good food, water, and plenty of sleep.
- Move your body regularly. Even one session of moderate-to-vigorous physical activity can reduce anxiety.
- Make time for friends and family. Talking with others can help you cope.
- Spend time in nature, it can help relieve stress and anxiety, improve your mood, and boost feelings of happiness and wellbeing.
- Engage in mindful meditation. Mindfulness can help you train your mind to be more focused, calm, and resilient. The UW-Madison Center for Healthy Minds developed a free Healthy Minds Program App that is rooted in research and uses two major modalities of learning that have been shown to result in lasting change.

Practice daily gratitude. Research has shown that practicing gratitude can lead to improved physical health, improved optimism and positive emotions, improved sleep, and reduced burnout. When we embrace and practice gratitude regularly, it has the power to measurably improve our overall happiness and well-being while also benefiting others.
- Know where to go for help.
It is essential to recognize that stress management is not the sole responsibility of the individual. Mitigating the negative effects of occupational stress by implementing proactive and protective measures is also the responsibility of the organizations in which we work. Occupational stress is often perpetuated by the organizational expectations, norms, and culture. For example, occupational stress is exacerbated when individuals perceive a lack of autonomy in their work and a lack of support from administrators and colleagues. Cultivating an organizational culture that truly supports and promotes employee wellbeing is essential to the health of the individuals in the workplace and to the health of the organization.
While some degree of occupational stress is likely inevitable, efforts should be made to reduce the amount of stress that individuals experience. It is encouraging that we have ample evidence-based strategies at our fingertips that can be implemented by both individuals and organizations to reduce stress and improve healthy coping. Arguably, employees are the greatest asset of any organization. By providing resources and support for individual wellness, organizations can foster a positive work culture and are intentionally investing in the wellness of every employee, which has the potential for limitless positive returns. Collectively, we have the opportunity to make a positive and lasting impact.

Licensees can update name or address information on the DSPS self-serve webpage. Please note that confirmation of change is not automatically provided.
Legal notices will be sent to a licensee’s address of record with the Department. Under s. 440.01(1), Wis. Stats., an applicant or credential holder has 30 days to provide notification of a change in name or address. Similarly, under s. 440.11(1m)(a), Wis. Stats. an applicant for or recipient of a credential who changes his or her electronic mail address or whose current electronic mail address becomes inactive shall notify the department of such a change within 30 days of the change.
