Washington State School Nurse Data Codes
It is time to update the Washington State School Nurse Data Codes. The codes were developed by Washington State school nurses and are updated periodically. They provide a common language to use in school health documentation.
The current list of codes can be found here. The codes attached were added last fall but have not been widely distributed. This is the file we will be starting with.
Angela Fry (afry@esd123.org), ESD 123’s Regional Public Health Coordinator, will be managing the change process.
If you have identified codes that you would like to add, change, or delete, please fill out the Change Request form that can be found at this link. The form is for one requested change only. If you have multiple requests, fill out multiple forms.
Please consider the following when submitting your request(s).
- Check to make sure you do not duplicate an existing code.
- Does this belong here? Is this appropriate for data set proposed (is this where this type of information should be recorded)
- Is consistent with school nursing and other nurse language sets (to learn from what others have decided to do)
- Is worded clearly – short and long descriptions
- Has a clear definition or description of how it will be used
- An argument as to why it should (or should not) be added.
- Please note, the following are rarely added so have a compelling argument as to why it is needed.
- Are we trying to work around program (PowerSchool, Skyward etc.) limitations?
- Is this a temporary need?
- Is this program-specific?
Once forms are received, volunteers will assist in reviewing proposed changes. If you are interested in volunteering, please complete the first part of the survey, even if you have no requests. You can also send Angela an email with your name, district and email address, afry@esd123.org.
If you have any further questions, please contact Angela at the email listed above. Thank you for taking the time to participate in this process.

DOH Health Needs Survey for Youth 13 to 22 Years
DOH is looking for youth ages 13 to 22 years living in Washington to take an anonymous online survey. The survey is available in English and Spanish and can also be made available in other languages whenever needed. The survey mostly has open-ended questions and should take participants about 15 minutes to complete. Survey questions ask young people about their last visit, how they think the visit went, any challenges they faced when accessing or attempting to access health care services, and any recommendations they have to improve health care for young people.
Please note: The intent of the survey is that it is completed by young people. Adults should not answer questions or fill out the survey for youth unless they need and request assistance.
We need your help to get this survey to young people and the organizations that serve them! Here’s how:

Recovery - Finding a Way forward After Crisis
A critical step in moving past crises and improving emergency response is listening to the people who were directly involved. What went wrong, what went well, why did it happen the way that it did, and what would we change next time?
On Wednesday, February 8th, Drs. Kira Mauseth and Tona McGuire will introduce us to this topic in an event called Recovery: Finding a Way Forward After Crisis. During this session, they will discuss the science of recovery and highlight the lasting benefits that these types of conversations can have for schools, educators, and staff.
