- To help you prepare for and expedite reporting, we are offering a downloadable Excel Enrollment Worksheet to complete prior to filling out the online form. This is only a tool and not required, but it should enable you to transfer your data in fifteen minutes or less.

- The State Board of Education is collecting demographic information for students in Kindergarten through 12th Grade only. However, we do ask that you report the total number of students in your Early Learning / Pre-K program, if you provide one.
The official count day for the private schools is October 3, 2022.
The enrollment data you provide should reflect the official school enrollment as of that date. Please complete and submit the enrollment form before the close of business on November 15, 2022.

The State Board of Education oversees Enrollment Reporting and Annual Approval. SBE also maintains up-to-date private school information. To access these three State Board of Education forms, your credentials are always:
- Username: Head of School Email Address
- Password: School Building Code
The Office of the Superintendent of Public Instruction oversees various other reporting requirements and schools will use the Educational Data system, or EDS, for those reports.

Username:
- The Report Form is case sensitive, so check to be sure that the email you enter is identical to the email used for your Approval Application last spring OR on your Change of Information Form.
- Double-check that the email address of your Head of School was typed correctly in your Approval Application or Change of Information Form. Even if you do not see a typo, try a copy-and-paste into the username field.
- If you have a new head of school since last spring, and you did not submit a Change of Information form before the September 23rd deadline, try using your previous Head of School email address. (You will be able to update this information once you have accessed the form.)
Password:
- Your School or Building Code is your password, and it always has four digits or a combination of three digits and one letter at the end.
- Again, the Report Form is case sensitive. If there is a letter at the end of your code, be sure it is capitalized.
- Some codes end with the letter O or I. If yours is one of those, be sure you enter the letter rather than the numerals 0 or 1.
You should have received an email confirmation and PDF of your Enrollment Report when you submitted your form. If you haven’t already, please take a few minutes to ensure:
- the total number of students in each grade equals the sum of the students in the Race / Ethnicity categories
- the total enrollment for the school does not include early learners or pre-k students
If you discover an error, please send a detailed email to the Private Schools Manager before November 15, and she will update your answers and send you the corrected enrollment form for your records.
If you did not receive an email confirmation with a PDF, please contact the Private Schools Manager.
Your Building Code is your password for all State Board processes. You can find your code using the EDS Directory Information to look up or confirm your unique code for each campus:

Log in to EDS as usual, then choose “Search Education Directory”
- Use the "Organization Category" dropdown box to select Private School.

- Use the "Organization Name" text field to enter the name of the school, then the result of that search will populate in the lower part of that page.
- On the left-hand side of the search results, you will see the name of the school followed by the school code in parentheses.
This code will function as your password for both the Enrollment Report and the Annual Approval Application, so please save it in a safe place.
