The Commission on African American Affairs, in association with Halcyon NW and Equity Leadership Collaborative invite you to participate in a statewide Black Health and Wellness survey.
We are concerned about the quality of life for American Americans in Washington state in the areas of economic stability, wellness, housing, wealth, education, child welfare, environmental and social justice.
Please complete our 15-minute survey and consider participating in our online focus groups. By sharing your lived experiences with us, we can continue to advocate for change.
Both the survey and focus groups are designed to protect your privacy.

Dr. Alexa Canady, the first African American woman in the United States to become a neurosurgeon, almost dropped out of college until a summer program in genetics for minority students helped her discover a desire to study medicine.
It was a career path that some advisers discouraged her from pursuing, and she encountered difficulty in obtaining an internship. With persistence, Canady was accepted as the first woman and African American surgical intern at Yale-New Haven Hospital. Canady specialized in pediatric neurosurgery - caring for young patients who faced life-threatening illnesses, gunshot wounds, head trauma and brain tumors. She became the chief of neurosurgery at the Children's Hospital in Michigan from 1987 until her partial retirement in 2001.
She was presented the 1993 American Medical Women's Association President's Award and in 1994 the Distinguished Service Award from Wayne State University Medical School.
Relocating to Florida with her husband in 2001, Canady learned there were no pediatric neurosurgeons in her immediate area and came out of retirement to practice part-time at Pensacola’s Sacred Heart Hospital. She fully retired from practicing medicine a second time in 2012.
