The Health Care Authority renamed the program for people enrolled in Dual-Eligible Special Needs Plans (D-SNP)! In Washington state, D-SNP is now called: Apple Health Medicare Connect.
This name change better communicates that Apple Health Medicare Connect is only available to clients who are enrolled in both Apple Health (Medicaid) and Medicare coverage (referred to as dual-eligible).
What is Apple Health Medicare Connect?
Apple Health Medicare Connect is a special Medicare Advantage (MA) plan for dual-eligible individuals aimed at improving care coordination for those receiving Medicare and Apple Health services. These special MA plans have a contract with the state of Washington to serve dual-eligible individuals.
