HCA was recently notified that Premera’s HMO plan did not meet the federal medical loss ratio (MLR) standard in 2025, as determined by the Centers for Medicare & Medicaid Services (CMS). Premera is required to send a CMS-standardized letter to SEBB subscribers who were enrolled in the Premera HMO plan in 2025 to notify them that they may receive a premium reduction from HCA. Premera must mail this letter by September 30, 2026.
Note: Premera estimates it will send the Notice of Health Insurance Premium Rebate letter to 3,326 subscribers who were enrolled in the Premera HMO plan in 2025. Not all letter recipients will receive a refund, and Premera HMO subscribers who enrolled starting in 2026 will not receive a letter from Premera (see below). This refund does not include SEBB subscribers enrolled in Premera High PPO and Standard PPO plans.
Next steps
HCA will issue a refund as a one-time premium reduction in December 2026 to current subscribers enrolled in the Premera HMO plan as of November 2026 and who continue their enrollment in December 2026. This will not include any subscribers whose enrollment in the plan ends on November 30, 2026. The premium reduction will be less than $3 per account for December 2026 coverage. Review the November billing file for the December 2026 premium reduction.
Subscribers do not have to take any action to receive the premium reduction. It will be automatically applied to their monthly premium amounts.
