Attention providers

What is changing?

The Health Care Authority (HCA) has revised the following billing guides to reflect billing requirements for collaborative care management (CoCM) services:

Physician-Related Services/Health Care Professional Services
Federally Qualified Health Centers (FQHCs)
Rural Health Clinics (RHCs)

Previously, FQHCs and RHCs used Healthcare Common Procedure Coding System (HCPCS) code G0512 to bill for CoCM services. However, the Centers for Medicare & Medicaid Services (CMS) deleted HCPCS code G0512 effective December 31, 2025. As a result, FQHC and RHC providers must use the remaining available CoCM billing codes when billing HCA for services provided to HCA clients.
