To become an Apple Health (Medicaid) billing provider, birth doulas must:
Enrolling as a billing provider
The birth doula billing and fee schedules are available on the HCA website.
Required documents for all Apple Health billing providers
Electronic Funds Transfer information and a signature is gathered during the enrollment steps in ProviderOne. Signatures on the listed documents are required for the form to be considered complete.
Doulas enrolling as an individual billing provider also need to:
- Obtain a business license
- Obtain professional liability insurance
- Enroll using their individual NPI
Doulas enrolling as a group or facility, agency, organization, institution (FAOI) billing provider also need to:
- Obtain a business license
- Obtain professional liability insurance
- Enroll using organizational NPI
The Health Care Authority (HCA) recommends having a copy of the Washington state birth doula certification, business license, and completed copies of all required documents prior to starting the ProviderOne application.
