Is your facility still using the outdated managed care enrollment form?

The Health Care Authority (HCA) moved to paperless processing of Apple Health (Medicaid) plan change requests in 2017. The Managed care enrollment form (HCA 13-862) was permanently deleted from our online forms library in November 2017. 

HCA is still receiving faxed copies of the outdated Managed care enrollment form from some health care facilities. We do not accept or process plan change requests submitted through any paper form. WAC 182-538-060 Managed care choice and assignment reflects this change.

What you need to do

We are asking health care facilities to:

  • Provide instructions to your staff on the how clients can change their managed care plans.
  • Update any internal processes to align with the options shared below.
  • Recycle and delete any stored versions of outdated Managed care enrollment form (HCA 13-862).
