DBHR's Discharge Planners Toolkit is now live!
The Discharge Planner's toolkit is an electronic decision tree that uses skip logic to assist Discharge Planners, case managers and other staff to assist in connecting individuals needing housing assistance to efficiently connect to housing resources in their community by:
- Identifying the items or resources individuals may need in place for a successful discharge or to help facilitate a warm handoff
- Helping staff working with the individual to get a handle on needed documentation and forms of ID, benefits that they have/will be eligible for, and identifying potential sources of income/wages
- Directly linking to forms/applications
This will ensure increased coordination between discharge planners in state institutions, hospitals, and treatment facilities with local housing networks/programs with the goal of decreasing the number of people becoming chronically homeless or discharging back into homelessness through:
- Providing people choice, power, and control over their lives through information and resources
- Documenting an individual’s homelessness and chronic homeless status to meet HUD requirements
- Warm handoffs of individuals to homeless assistance programs/housing networks e.g. Coordinated entry
