The Health Care Authority (HCA) is aware that the Sterilization Consent form (HHS-687) lists an expiration date of 4/30/2022.
The Centers for Medicare and Medicaid Services (CMS) is working on the form update. The current form is temporarily approved for an additional 30 days, through May 30, 2022. Please continue to use this form.
As soon as the updated form is published on the U.S. Department of Health & Human Services website, under Key Resources for Title X Grantees | HHS Office of Population Affairs, HCA will notify providers.
