Updated: New hysterectomy consent form now available

The Health Care Authority (HCA) has received much feedback that our previous form was problematic and led to claim denials. In response we have revised the form - improving both clarity and accessibility.

Important: Only the new form will be accepted beginning January 1, 2020.
