Good afternoon Foundational Community Supports (FCS) community!

DBHR’s FCS team needs you!

We are looking to our FCS providers to provide us with your Supportive Housing and Supported Employment success stories so that we can continue to share the great work you are doing serving folks across Washington State.

Here is what we would like:

One to two paragraphs with your participant’s FCS success story ( please leave participant names out)
Please include agency Name and county you serve.
You may also include a picture of agency, FCS team, or participant with prior release.
We will accept success stories at any time of course BUT would greatly appreciate some submitted prior to COB of July 8th, 2019
