Apple Health (Medicaid) Provider Alert

What is changing?

Effective for dates of service on and after July 1, 2019, the Health Care Authority will begin paying publicly owned or operated ambulance providers for services provided pursuant to community assistance referral and education programs. To receive this payment, qualifying providers must complete and submit the HCA Treat & Refer Program Participation Attestation (HCA 60-0024) form to the Health Care Authority.

Where do I find the form?

The HCA Treat & Refer Program Participation Attestation form will be available on the Health Care Authority (HCA) Forms & Publications page by June 3, 2019. This document must be returned to Provider Enrollment, via email, at: providerenrollment@hca.wa.gov.

Completion of this form lets HCA know which qualifying, publicly owned or operated providers have a community assistance referral and education program. Upon receipt of the completed form, HCA will add a Treat & Refer provider indicator to the provider’s profile which allows for payment.

If a provider chooses not to submit a completed form for participation for these services, HCA will not add a provider indicator to the provider’s profile and the provider will not be able to receive payment for treat and refer services.
