The Healthier Washington Medicaid Transformation team, which includes staff from HCA, Department of Health, and Department of Social and Health Services, were celebrated by Governor Jay Inslee at an employee recognition event on June 19.

Part one of Healthier Washington sustainability plan now online

Part one of Healthier Washington's plan for advancing health systems transformation is now available online.

The State Innovation Models (SIM) Sustainability Plan outlines Washington State's vision, accomplishments, changes in environment and lessons learned since the Healthier Washington initiative began.

The report provides a snapshot of the strides our state has made toward the Quadruple Aim of healthier populations, smarter spending, better care, and provider satisfaction.

New documents offer guidance for regions moving to integrated care

The Healthier Washington integration team has created two new resources for regions embarking on integration of physical and behavioral health.

HCA advises each region to create a communications workgroup to develop and disseminate information to Apple Health clients, and an Early Warning System workgroup to track issues and provide feedback on matters related to transitioning.

Health information technology resources added to website

The Healthier Washington Medicaid Transformation resources page now includes a section on Health Information Technology (HIT). The new menu item can be expanded to show the HIT Strategic Roadmap, operational plan, plan updates, and technical assistance documents.

These resources help shed light on how Accountable Communities of Health and the State are advancing HIT through Medicaid Transformation projects.

Tool describes steps for promoting chronic disease management

In the new world of integrated care, behavioral health providers are becoming increasingly important in their clients' chronic disease management strategies. While the symptoms and treatment of chronic diseases can vary, there are some common steps that behavioral health providers and care team members can take to assist clients in understanding, accepting, and managing their chronic disease.

A new tool at the Healthier Washington Practice Transformation Support Hub web portal describes these steps. It includes an extensive list of physical health resources that can help increase the care team's knowledge and confidence in discussing all aspects of a client's care, from understanding a diagnosis to discussing medication.
