Apple Health (Medicaid) Provider Alert

An updated Physician-Related Services Billing Guide, effective January 1, 2018, is available on the Provider billing guides page.

What has changed?

The Attestation for Collaborative Care Model form was listed with the wrong form number (HCA 13-007). The guide has been updated to reflect this correction and the form is now listed as (HCA 13-0017).
