Integrated Settings Provider Self-Assessment and Attestation
Dear Providers,
Providers are required to comply with the federal Integrated Settings requirements per 42 C.F.R. Section 441.301(c)(4), WAC 388-823-1095, WAC 388-823-1096, RCW 71A.26.030, and Policy 5.06. To comply with this requirement, providers evaluate their service provision in accordance with the federal Integrated Settings requirement by completing a self-assessment and attestation form DSHS 27-261 Integrated Settings Provider Attestation. This form is available online on the Electronic DSHS Forms site.
Form DSHS 27-261 Integrated Settings Provider Attestation is due Monday, Sept. 15, to your resource manager.
