Updated Positive Behavior Support Plan Template
Dear Provider,
The Developmental Disabilities Administration has updated the functional assessment and positive behavior support plan template and instructions. These are optional tools for providers to use.
This template is part of the updated training for plan writers. If you write functional assessments and positive behavior support plans, contact your regional clinical team’s Field Services psychologist or psychology associate for information on how to register for plan writer training.
