Dear Licensed Health Care Provider,
For those of you who may be eligible for COVID-19 vaccine in phase 1A according to the Department of Health Vaccine Prioritization Guidance, we are sharing information about statewide locations with vaccine inventory.

Some of you have already received your first dose of COVID-19 vaccine, and some may even have gotten both doses. Thank you for protecting yourself and your community from this deadly disease. However, many more of you have not yet had a chance to get your vaccine and we know you are anxious to do so.
A lot of health care facilities are enrolled COVID-19 vaccine providers. If you work for an enrolled provider, please talk to your employer to find out when you can receive your vaccine. For those of you who do not work for an enrolled provider, there are open vaccination places you can go. We have attached a list to this email of COVID-19 vaccine providers who currently have vaccine on hand and may be vaccinating people who qualify in phase 1a. You can reach out to the place or places nearest you to find out your options.
A clinic may require you to bring in a voucher. Learn more here about the voucher system. Or, visit www.findyourphasewa.org , and find provider scheduling information after completing the short questionnaire. If one facility is unable to accommodate you, please be patient with the high volume of requests they are receiving and try another facility.
