disciplines health care providers
OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department’s Health Systems
Quality Assurance Division works with boards, commissions, and advisory
committees to set licensing standards for more than 80 health care professions
(e.g., medical doctors, nurses, counselors).
Information
about health care providers is on the agency website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department
of Health website (doh.wa.gov). The
site includes information about a health care provider’s license status, the
expiration and renewal date of their credential, disciplinary actions and
copies of legal documents issued after July 1998. This information is also
available by calling 360-236-4700. Consumers who think a health care provider
acted unprofessionally are encouraged to call and report their complaint.
King
County
In March 2018 the secretary of health ended
conditions on the provisional certified home care aide and certified home care
aide credentials of Jessa Carrin Southland (PV60644820, HM60612021).
In March 2018 the secretary of health ended
conditions on the massage therapist credential of Yanling Wang
(MA60342152).
Pierce
County
In March 2018 the Pharmacy Commission conditionally
granted a pharmacist intern credential to Shayne Kikaha Fontes (IR60792089).
In 2016 Fontes was convicted of third-degree theft. In 2017 he took a necklace
from a store without paying for the necklace.
In March 2018 the secretary of health ordered Janise
West to cease and desist from practicing massage without a license, and to
pay a $1,000 fine. West, who has no massage license, advertised massage
services.
Skagit
County
In March 2018 the Emergency Medical Services and
Trauma Systems Program filed an amended statement of charges against emergency
medical technician Beau James France (ES60197294). The amended charges indicate
that in 2018 he was convicted of two counts of first-degree possessing
depictions of minors engaged in sexually explicit conduct.
Spokane
County
In March 2018 the Nursing Assistant Program filed an
amended statement of charges against registered nursing assistant Sabrina L.
Campbell (NA00120015). The amended statement alleges that at an adult
family home Campbell owned, she attempted to assist a vulnerable adult patient
in a manner that didn’t demonstrate sensitivity to the patient’s needs, and
without first assessing the resident’s mental or physical state.
In March 2018 the Veterinary Board charged
veterinary technician Lisa Dawn Ensworth (AT00001187) with
unprofessional conduct. Ensworth allegedly didn’t supply documentation of
having fulfilled continuing education requirements.
In March 2018 the secretary of health conditionally
granted a chemical dependency professional trainee credential to Ryan
Christopher Kunz (CO60656341) and ordered him to participate in a substance
abuse monitoring program. In 2004 Kunz was convicted of manufacture of a
controlled substance – methamphetamine. In 2014 he was convicted of possession
of a controlled substance – methamphetamine.
In March 2018 the secretary of health conditionally
granted a chemical dependency professional trainee credential to Matthew
Patrick Daul (CO60812110) and ordered him to participate in a substance
abuse monitoring program. In 2000 Daul was convicted in Arizona of theft, and
possession of dangerous drugs. In 2016 he was convicted in Washington of
driving under the influence, and obstructing a law enforcement officer.
Whatcom
County
In February 2018 the secretary of health granted a
one-year extension for the apprentice dispensing optician credential of Jennifer
Nicole Rusling (DT60245048). Rusling’s non-renewable license expired in
2017. Rusling was unable to become a licensed dispensing optician within six
years because of circumstances beyond her control.
In March 2018 the Dental Commission ended conditions
on the dental assistant credential of Stephanie Nichole Snyder
(D160306396).
Yakima County
In February 2018 the Dental Commission denied a
dental assistant credential to Nancy Vazquez (D160576576). In 2011
Vazquez was convicted of prostitution. In 2013 she received a deferred sentence
on a prostitution charge.
Out
of State
California:
In March 2018 the Medical Commission entered an agreement with physician Andrew
L. Kominsky (MD00046940) that fines him $5,000, places conditions on his
practice, requires him to complete a boundaries course, mandates that he receive
psychotherapy, and directs him to write a paper. Kominsky had an inappropriate
relationship with a cancer patient who subsequently died.
Note to Editors: Health care providers charged with unprofessional
conduct have 20 days to respond to the Department of Health in writing. The
case then enters the settlement process. If no disciplinary agreement can be
reached, the case will go to a hearing.
