State revokes,
suspends licenses, certifications, registrations of health care providers
OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems
Quality Assurance Division works with boards, commissions and advisory
committees to set licensing standards for more than 80 health care professions
(e.g., medical doctors, nurses, counselors).
Information
about health care providers is on the agency’s website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department
of Health home page (doh.wa.gov). The
site includes information about a health care provider’s license status, the
expiration and renewal date of their credential, disciplinary actions and
copies of legal documents issued after July 1998. This information is also
available by calling 360-236-4700. Consumers who think a health care provider
acted unprofessionally are also encouraged to call and report their complaint.
Clark
County
In August 2017 the Nursing Assistant Program charged
registered nursing assistant Chelsey Rae Atkins (NA60523460) with
unprofessional conduct and immediately suspended her credential. After finding
that Atkins neglected a vulnerable adult in her care, the state Department of
Social and Health Services will not allow her to be employed in caring for or
having unsupervised access to vulnerable adults.
In August 2017 the Nursing Assistant Program charged
certified nursing assistant Maria E. Romero (NC10074012) with
unprofessional conduct and immediately suspended her credential. The state
Department of Social and Health Services, which found that Romero neglected a
vulnerable adult in her care, will not allow her to be employed in caring for or
having unsupervised access to vulnerable adults.
Grays
Harbor County
In August 2017 the secretary of health revoked the
home care aide credential of Tammy Irene Dalton
(HM60456277) after the state Department of Social and Health Services
disqualified her from being employed in caring for or having unsupervised
access to vulnerable adults.
Pierce
County
In July 2017 the secretary of health indefinitely
suspended the certified medical assistant-hemodialysis technician credential of
Kariann Lynne Williams (HT60370014), who didn’t respond to a Department
of Health investigator’s inquiry about a complaint alleging she accepted
narcotics from a patient.
Stevens
County
In July 2017 the secretary of health suspended the
home care aide credential of Denise Franceen Hager (HM60607052) for at
least three years. Hager used a vulnerable adult patient’s debit card to make
unauthorized purchases for her own use.
Whatcom
County
In July 2017 the secretary of health suspended the
registered nursing assistant credential of Lawrence Edward Larsen
(NA60272354) for at least five years. In 2016 Larsen was convicted of
second-degree assault.
Out
of State
Oregon:
In July 2017 the Nursing Commission indefinitely suspended the registered nurse
credential of Jonathan Peter Tannler (RN60183136), who didn’t comply
with 2016 reinstatement conditions involving performance evaluations, progress
reports, and continuing education.
Texas:
In August 2017 the Nursing Commission indefinitely suspended the
registered nurse credential of Veronica Azucena Benavides (RN60496794).
Benavides’ Texas nurse license was suspended in 2017 after she failed to comply
with conditions on her license regarding substance abuse monitoring and ethics
classes.
Virginia:
In August 2017 the Nursing Commission charged registered nurse Myra Jo
Easter (RN60687119) with unprofessional conduct and immediately suspended
her credential. Easter’s Virginia nurse license was suspended in 2017 after she
was dismissed from a monitoring program.
