Action Required: Annual Vocational Firm Agreement Due Dec. 15, 2025

Attention all vocational firm owners and managers receiving State Fund referrals:

The deadline to submit your Annual Vocational Firm Agreement is Dec.15.

  • Fax all three pages of your signed Annual Vocational Firm Agreement, with all required documentation, to Private Sector Rehabilitation Services at 360-902-6706 by Dec.15.
  • Remove personal information from all documents, such as home address and other covered drivers, before sending.
  • You may submit photos or scanned copies of credentials and/or insurance information.

To receive referrals from L&I, you must meet the requirements outlined in the agreement for each of your service locations.

Please note these important points:

  • Provide L&I with up-to-date evidence of insurance.
  • Send credential verification for any provider whose credential is expired or is expiring as of Dec. 31, 2025. Firms are also encouraged to submit credential verification throughout the calendar year, if relevant.
  • L&I will not require credential verification for any provider who has already submitted up- to-date credentials that are valid through Dec. 31, 2026.
  • All required attachments must comply with state law.

By signing the agreement, you commit to its terms and all applicable federal and Washington state statutes, rules, and policies.
