DUE JULY 31: Intern Supervisor Expectations Form

Attention All Firm Managers: Send to all Intern Supervisors

This is a reminder to submit your updated Vocational Intern Supervisor Expectations Form by Thursday, July 31. Send the completed forms to Private Sector Rehabilitation Services at PSRS@Lni.wa.gov.

The form must be completed and sent to L&I every six months during a counselor’s internship. The form outlines the skills needed to meet the expectations of completing the internship. The supervisor confirms the intern understands and demonstrates each skill before initialing. This requirement only applies to interns assisting with State Fund referrals.

The intern supervisor should:

  • Complete and sign the form, along with the intern’s signature.
  • Initial and date each skill the intern has successfully demonstrated.
