The Department of Labor & Industries (L&I) has posted the following fee schedule code and policy updates for January 2025:

Increased Maximums for Job Modifications

Procedure codes 0380R and 0385R are increased to a maximum allowable of $10,000 per job or job site.

29914 and 29915 are Covered with Utilization Review

Procedure codes 29914 and 29915 are covered when hip labral tear co-occurs with femoroacetabular impingement (FAI) syndrome. Utilization review is required. These codes aren't covered for FAI surgery without hip labral tear, per coverage decision. Both codes are part of endoscopy family 29860.

Dry Needling

Physical therapists (PTs) with proper endorsements can now practice intramuscular needling. See our Updates and Corrections page for details.

Skill Enhancement Training

Workers who qualify may choose to use this incentive when they are in a qualifying vocational referral and have been assigned a vocational rehabilitation counselor. See our Updates and Corrections page for details.

Taxi Trip Tickets

Taxis are required to submit form F248-374-000 in addition to their bill. This form takes the place of a standalone report. See our Updates and Corrections page for details.

Mobile Clinic Services

A new payment policy for services provided via mobile clinic is now available. This policy is not applicable to providers who are traveling outside their credentialed service location. Providers performing services must have an active provider account number under the mobile clinic. See our Updates and Corrections page for details.

Report of Accident/PIR Clarifications

Updates have been made to the Report of Accident (ROA) payment policy to include clarifications regarding when basic first aid and when an ROA is required. See our Updates and Corrections page for details.

Post-Exposure Prophylaxis (PEP) Drugs for HIV Dispensed from Emergency Department

A new payment policy for PEP HIV drugs direct dispensed from an emergency department is now available. See our Updates and Corrections page for details.
