Department developed templates must be used by the Self-Insured Employers and Third-Party Administrators to inform workers of actions taken on claims involving the delivery of benefits. Learn what forms and templates you must use when communicating with the department, workers, and providers during the course of the claim. Failure to follow RCWs and WACs may lead to a penalty.
