Hello Child Placing Agency (CPA) partners,

Required Documents

The required documents booklet has been separated into two booklets, one for Kinship and the other for Foster Care. Please see the updated versions on the CPA Resources and Training Hub webpage.

Caregivers and Direct Deposit 

Caregivers who don’t use direct deposit must complete and submit a provider registration form by May 1, 2026. Caregivers have two options for submitting the forms: 

• E-sign forms using Docusign (fastest method)
• Print and sign, then email forms to ProviderFileUnit@dshs.wa.gov 

This communication will be in the Caregiver Connection through May 2026. The DCYF webpages will be updated in the coming months, and CaRES has been notified. 

Additionally, the direct deposit forms listed on the SSPS Main Menu with the revised date of Oct. 17, 2023, are outdated, and OFM will stop accepting them at the end of this year.

Ensure caregivers are using the forms directly from the OFM website: Statewide Vendor/Payee Services. Check out the updated webpages for step-by-step instructions.
