The 2024 MAPP2Health Community Assessment is underway! If you live in Charlottesville, Albemarle, Fluvanna, Greene, Louisa, or Nelson, this is your chance to share what matters most about your health. We’re inviting residents Districtwide to participate in focus groups—virtually or in-person—to talk about the challenges that impact their health and the health of their communities. Focus groups are available in five languages. Focus group participants receive a $25 Food Lion gift card. Complete the focus group interest form and our CHA/CHIP Program Officer will contact you with scheduling opportunities.
Complete the Stakeholder Survey
Are you part of an organization, agency, or coalition that serves local residents? We are looking for your insights into what factors are impacting the health of your clients, patients, and communities. The Stakeholder Survey will be open until year’s end.
Both the focus groups and the Stakeholder Survey will help organizers BRHD, Sentara Martha Jefferson Hospital, UVA Health, and our community partners better understand what residents across the district need to live healthier lives—and how we can better support those needs. We’ll publish the results of our findings in the September 2025 MAPP2Health Report.

In addition to some wonderful training opportunities, Transformative Changes will be co-hosting a virtual Community Health Worker Town Hall on November 7 from 12:00 to 2:00pm via Zoom. This Town Hall will provide an opportunity to request CHW Listening Session themes, brainstorm solutions to address the needs of CHWs, and connect with allies working to amplify the aspirations of CHWs.
