NEW Athletic Registration for 26-27

We are very excited to announce that Fairfax County Public Schools (FCPS) Student Activities and Athletics has partnered with FinalForms, an online forms and data management service, for student registration for athletics and activities.

FinalForms allows you to complete and sign athletic forms for your students online. FCPS and FinalForms have worked closely to ensure that your most sensitive information is safeguarded and meets all of FCPS’ rigorous data protection requirements.

Key Benefits of FinalForms:

  • Saves Time: FinalForms saves data year-to-year, meaning you will never need to enter the same information twice.
  • Pre-populates Data: Information will be pre-populated wherever possible for each of your students.
  • Easy Updates: You can review your data at any time to verify it is current. You will only be required to sign your forms once per year and after any updates.

What You Need to Do:

  1. Check Your Email: All parents with students registered in grades 9-12 will receive a confirmation email from FinalForms delivered to the email address associated with the enrolling parent.
  2. Check Spam: Please check your spam/junk folder. If the email is there, please "mark as safe" and add FinalForms to your contacts. Annandale and the Office of Student Activities and Athletics will use this platform to communicate regularly with you throughout the school year.
  3. Get Started: Once you receive the initial email, please review the Parent Playbook Step By Step Guide to get started.

Parent Registration - How do I get started?

  1. Check your email for an ACCOUNT CONFIRMATION EMAIL from the FinalForms Mailman. Once received and opened, click CONFIRM YOUR ACCOUNT in the email text.
  2. Create your new FinalForms password. Next, click CONFIRM ACCOUNT.
  3. Your account will be confirmed, and you will be logged in.

What if I did not receive a confirmation email?

  1. Go to: https://fairfax-va.finalforms.com/
  2. Underneath the Parent Login icon, select the blue CLICK HERE FOR A CONFIRMATION EMAIL link.
  3. Enter your email address and click the blue SEND SECURE LINK button. If your email is registered on this FinalForms site, you will receive a confirmation email and see a confirmation message at the top of the page. If your email is not found, an error message will appear at the top of the page. In that case, please contact your school administrator to update it.
  4. If you're unsure of your email address, click FORGOT YOUR EMAIL to search for your name and select it from the dropdown menu. If your name is not listed, please contact your school administrator to update your email address.

Registering a Student - What information may I need?

  • Basic medical history and health information
  • Your insurance company and policy number
  • Your doctor, dentist, and medical specialist contact information
  • Your hospital preference
  • Emergency contact information

How do I register my first student?

  1. Go to: https://fairfax-va.finalforms.com/
  2. Click LOGIN under the Parent icon.
  3. Locate and click the INCOMPLETE FORMS button for the student you wish to register.
  4. If your student plans to participate in a sport, activity, or club, click the checkboxes for each. After making your selection, click UPDATE. You may change your selections until the registration deadline.
  5. Complete each form and sign your full name (i.e., Jonathan Smith) in the parent signature field on each page. After signing each, click SUBMIT FORM and move on to the next form.
  6. When all forms are complete, you will see a "Forms Finished" message.

IMPORTANT: If your district requires it, an email will automatically be sent to the email address you provided for your student, prompting your student to sign the required forms.


How do I register additional students?

Click MY STUDENTS. Then, repeat Steps 3 through 7 for each additional student.


How do I update information?

Log in at any time and click UPDATE FORMS to update information for any student.

Note: The final section says to "repeat Steps 3 through 7" for additional students, even though the first registration instructions only list Steps 1 through 6. This appears to be a minor numbering error in the original document.
