After-School Program Survey
The Middle School After-School Program office is conducting a survey of parents/ guardians to assess the quality and impact of the program. Below is a link for a quick survey about the Middle School After-School Program. The survey should take no longer than 5 minutes to complete.
Participating in the survey is voluntary. Individual responses will be kept confidential, and responses will be summarized at the school and division level.
The survey is an important component of the after-school program evaluation and is critical for future funding. The more responses we receive, the more valuable and reliable the data
Thank you for considering completing the survey and for your support of the MS after-school program. This survey will be open from June 8, 2026 through June 12, 2026.
Please click HERE to complete the survey Â
If you have any questions about the survey, please contact Mark H. Emery at mhemery@fcps.edu.
You should exit the FCPS survey once the “Thank You for Completing this Survey” appears. The request to click on a second survey is not part of the Parent Survey. Thank you

Expunge Form
Middle school offers the unique opportunity to expunge a high school credit course (HS math courses and World Language Courses). This means that there would be no record of this year’s grade on their high school transcript. Please review your child's third quarter grades and contact your child's school counselor for any questions. If you would like to start this process, here is the link to the Expunge Form.

🏨Important Information from Cooper Clinic
Important Reminder
The school health room is reminding parents and guardians that all medication stored in the health room must be picked up no later than the student dismissal time on the last day of school. Medication left in the health room after that time will be destroyed. It is school policy that medications must be picked up by the parent or guardian. Medication cannot be sent home with your student. However, with parent or guardian permission, a high school student may transport over-the-counter medications to and from the school health room. If your student will be enrolled in a summer program sponsored by Fairfax County Public Schools, the existing medication authorization form(s) may be used for the summer program, but you will need to pick up the medication on the last day of school and deliver the medication and completed forms to the summer learning site on the first day of the summer program. Please let the health room know if you would like a copy of the form(s).
