School Forms:

✔️Have You Completed These Important Forms?

The multiple school forms parents and caregivers receive during the first weeks of school can feel overwhelming, but each one helps set your child up for a successful school year.

If you do not have a SIS ParentVUE account, please contact your child’s school for an activation code and follow the directions on the SIS ParentVUE Account webpage

🩹 Immunization Requirements

Please check with your health care provider as soon as possible to make sure that your child is up to date on the immunizations required by the state of Virginia. Starting Tuesday, November 18, students who have not provided proof of immunization may not be allowed to attend school.

You may check your child’s immunization compliance and submit their immunization record in SIS ParentVUE. Records may also be delivered to your school’s front office. Find more information on immunizations

📋 Permission Forms

Families can easily grant permission for their children to access a variety of school resources through the Parent Digital Consent system. Parents can opt their child in or out of resources, including online educational tools, school counseling services, and tutoring services. They can also change how student information is managed.

Visit the Parent Digital Consent webpage to access the system. Watch a video to see how to use the tool

Student Rights and Responsibilities (SR&R)

The SR&R document explains expectations for student behavior and adult responses. Parents using SIS ParentVUE will be prompted to sign the document when logging in to that platform. A printable version of the document with a signature sheet is available on the SR&R webpage. This form should be signed and returned to your school’s front office by Tuesday, September 30.

Fall Full-Time AAP Screening:

Parents and teachers interested in referring students for the fall full-time AAP screening must submit a full-time AAP referral form to the school’s Advanced Academic Resource Teacher (AART) by October 15, 2025. Fall 2025 screening is only available to students newly enrolled in FCPS after January 1, 2025, who were not screened in Spring 2025.

For information pertaining to the screening process, testing and identification timeline or identification and placement, please contact Laura Ballou at liballou@fcps.edu.

Student Buss Pass:

We’d like to remind you of our school’s policy regarding students riding home on the bus with a friend or classmate.

If your student plans to go home with another student on a different bus route, we must receive a written note from both students’ families in order to issue a bus pass. This is to ensure the safety and accountability of all students using school transportation.

Both students will be required to present their bus passes to the bus driver before boarding.

We appreciate your cooperation in helping us maintain a safe and organized transportation process. If you have any questions, please contact the main office.

Red Bag Event:

Our school is partnering with Food For Neighbors to help feed our local students who deal with food insecurity outside of school hours. Even in an area as wealthy as Northern Virginia, one in four kids are struggling with hunger.

Food For Neighbors is a local organization addressing this problem by collecting food from our community via the Red Bag Program and donating directly to our local schools. The food is then distributed to the students every week through our food pantry. We hope you will consider participating in the Red Bag food collection program. You can make a difference in a local student’s life by simply adding a few extra items to your shopping cart every two months.

Here’s how it works:

1. Request a reusable Red Bag here and Food For Neighbors will drop it off at your house.

2. Fill the Red Bag with food items that are listed on a tag on that bag.

3. Put the Red Bag outside your front door in the morning on the next bag collection day. Collection dates are also listed on the bag tag. And don’t worry, FFN will send reminders.

Your Red Bag will be picked up from your house, the food sorted at a local school and delivered directly to our school food pantry.

If you have any questions, please feel free to contact Student Services or our Family Liaison.
