Student Intent Form

Please let us know if your child/children will be returning to Oak Hill next year. Please fill out this Google form for each of your children currently attending Oak Hill ES. Sixth grade parents should also fill out the form and indicate which middle school their child will be attending.

Please complete the form by April 10th. This information will help ensure that we are appropriately staffed for next school year.
