IMPORTANT -
Health Information Form- check your student’s backpack
For those students still missing the form, we have sent home the form in your student’s red folders. Please complete and return the form to school by Friday, October 25th.
The Health Information Form (HIF) provides valuable information regarding student health conditions. These forms are critical in the early identification of students who may need healthcare support in the educational environment. Along with the development of Healthcare Plans (HCP) and Action Plans, the Public Health Nurses (PHNs) utilize the information contained in the HIF to develop teaching strategies for FCPS staff in order to meet the requirements of Regulation 2104 Health Services-School Health-Related Needs.
These forms are required for all students. Enrolling parents/guardians who have a SIS ParentVUE account may submit them electronically by using the Online Verification/Update packet.
Federal Impact Aid Survey
Attention FCPS Families!
Return Your Federal Impact Aid Survey
FCPS is asking all families to fill out a Federal Impact Aid Survey. The form is available online in SIS ParentVUE.
Click here for instuctions : https://www.fcps.edu/impact
For any families who have not completed the survey in ParentVUE, we will provide paper forms starting Wednesday, November 13. Paper forms should be returned by Wednesday, December 4.
Completing this form for each school-aged child in every household is important, because information from this survey resulted in more than $4 million in funding last year from the U.S. Department of Education. These funds are used to support all Fairfax County public schools and centers.
Please watch this video and visit the Federal Impact Aid Survey page for more information.

Learn how to activate your SIS ParentVUE account, so you can easily submit your survey.

Veteran's Day
Veterans Day is a time to honor all who have served in the United States Armed Forces. It is a day to reflect on the sacrifices they have made and to express our gratitude for their service. Students play an important role in honoring veterans on Veterans Day. If there is a veteran in your life (family member, friend, neighbor, etc.) who you'd like to recognize, please fill out the form that you will find in your student’s backpack today. Please include a photo of the person that you would like to honor (a photocopy is preferred, as originals won't be returned). These forms will be displayed in our hallway as a reminder to our students and visitors of the importance of honoring our veterans. If you have more than one family member you'd like to honor, please feel free to make copies of the form. All forms with pictures should be submitted to the Key Center main office by Wednesday, November 6. You may also attach the completed form with a photo by emailing them to becurtis@fcps.edu.
