Health Information Forms and Emergency Care Forms

We want to remind you of the importance of the Health Information Form (HIF) and the Emergency Care Information form for your child’s enrollment. These forms are essential for identifying students who may need healthcare support within our educational environment.

If you have a SIS ParentVUE account, you can submit these forms electronically through the Online Verification/Update packet. For families without a SIS ParentVUE account, these forms are available on the General Registration Forms page. If you need a ParentVUE account, please contact Victoria Reyes at vvreyes@fcps.edu.


State of Attendance for Quarter 1

Liberty Middle School will again send out periodic updates on attendance data throughout the year. Before the end of the quarter, caregivers will receive an update with the number of absences their student has had so far this year as well as tardy data. All students’ caregivers who enrolled their student will receive this data, and it is not in any way meant to be a warning or an alarm—just an FYI. It will include resources to support your students should that be beneficial or needed.

MWEE Chaperones!

Greetings!

We need chaperones to help with our MWEE (Meaningful Watershed Educational Experience). This year, Liberty Middle School 7th graders will be attending a watershed field experience on site. The experience is scheduled for Thursday, October 17, 2024 an Friday, October 18th, 2024. There will be 4 sessions over the 2 days, an AM session from 7:30-10:50 and the PM session from 11:20 until 2:10. Your student will attend on their corresponding science day.

Cross Country Volunteers

On Saturday 10/19 and 10/26, Liberty will be hosting cross country meets for the county. The meet will run in 2 stages from 8:00AM to Noon.  Your help is greatly appreciated supporting this event.  

The first groups will run from 8:00AM to 10:00AM and the second group of schools from 10:00AM - 12:00PM.  We need 18 volunteers around the course and to help lead students on bike as the Rabbit (ahead of students) and the Caboose (follow the last ones in).   Your help with this momentous event is greatly appreciated as we welcome over 200 students from across the county to Liberty.

Music Department Chipotle Fundraiser!

The Liberty Music Department will have its Chipotle Fundraiser on Tuesday, October 29. Please consider supporting our music students and enjoy a night without cooking! Thank you all so much for your support!

Liberty Library Amazon Wishlist:

Please consider helping the Liberty Library by purchasing off our Amazon wish list. Students love to use these items in the library makerspace and during lunches. Additionally, if you have MagnaTiles or PicassoTiles that your family is no longer using, I would love them for the library!

Materials Review Committee

We are seeking parent volunteers to serve on our Materials Review Committee. Committee members will read or watch the texts that teachers submit for approval to ensure that it is age appropriate for students. We will also meet monthly to discuss the texts prior to completing the required forms.

Free and Reduced-Price Meals: 

Learn more about the criteria and process to apply for free or reduced-price breakfast and lunch.

If you have children eligible for the federal Free and Reduced-Price Meals program, they may also be eligible to participate in other Fairfax County Public Schools (FCPS) fee-based programs without paying a fee or by paying a reduced fee (e.g fees associated with elective classes).

The free and reduced-price meals application also includes a section where you provide consent to share information allowing FCPS staff (School Principal, school counselors and other FCPS program staff) to share information about your children's meal eligibility status.
