Summer Office Hours

Herndon Middle School's summer office hours are 9:00 am to 1:00 pm. The building will be closed July 3rd and 4th in observance of the holiday and offices will also be closed July 5th-7th due to summer cleaning. Please plan accordingly and enjoy your summer!

Please join Herndon Middle School's PTA on August 17th from 3:30-5:30pm in the back field for a Social Gathering. We will have Food, Free Ice Pop and Music! 

Moving Over the Summer?

If your student will not be returning to Herndon HMS for the 2023-2024 school year, please complete the Student’s Intention for the Next School Year form.  You can email the completed and signed form to the Herndon MS registrar, Susanna Fry at smfry@fcps.edu. 

Immunization Requirement Notice

Immunizations

 

The Code of Virginia requires all students to be immunized against certain communicable diseases prior to entrance into Fairfax County Public Schools (FCPS). ACTION REQUESTED: Please provide immunization documents to Ms. Fry at smfry@fcps.edu before the start of the school year.

Immunizations  are available from your child’s healthcare provider, the links below to identify resources:

 

2023-24 Epinephrine and Asthma Medication Forms

Will your student have Epinephrine or Asthma medication in the HMS Clinic for the 2023-24 school year?

Be sure to bring the following to the clinic (drop off dates will be announced at a later date. Stay tuned):

  1. Complete the FCPS Medication Authorization forms General Registration Forms | Fairfax County Public Schools (fcps.edu). Please ensure to have your health care provider complete the updated FCPS Authorization for Anaphylaxis Action Plan (SS/SE-64) dated 05/2023 for epinephrine or the FCPS Authorization for Virginia Asthma Action Plan (SS/SE-65) dated 07/2022 for asthma rescue medication.  We no longer accept the old version of Inhaler Authorization form or Epinephrine Authorization form.
  2. Parents will need to provide the school with properly labeled containers (your pharmacist will usually provide an extra container if you request it). Only a 30-day supply of medication should be brought into school at a time.
