Student-Parent Survey for Federal Impact Aid

Impact Aid programs and grants are designed to assist local school districts that enroll federally connected students. The grants from these programs will become part of FCPS' general operating budget and are critical to ensuring that all children in FCPS receive an excellent education. FCPS uses the Student-Parent Survey for Federal Impact Aid to identify federally connected students and to support our request for Impact Aid resources. On Thursday, October 20, the survey will be distributed to all FCPS families. Please look for the Impact Aid survey in your child's Thursday folder on October 20. 

For each school-aged child in your household, please complete, sign, date, and return the survey by November 4. All results are confidential. 

If you are US Military: complete Section 1 and sign and date the bottom of the form:           

  • Parent/Guardian Name [Last, First, MI]
  • Rank or Grade
  • Branch of Service
  • Mobilized by Presidential Executive Order 13223 / Title 10 – [attach copy of activation orders]

If you are Foreign Military, complete Section 1 and sign and date the bottom of the form:

  • Parent/Guardian Name [Last, First, MI]
  • Rank or Grade
  • Name of Foreign Government
  • Branch of Service

If you work on Federal Property (Civilian), complete Section 2 and sign and date the bottom of the form:

  • Parent/Guardian Name [Last, First, MI]
  • Company or Agency Name (Employer)
  • Check the Federal Property where parent or guardian reported to work
  • Provide the full worksite name and address for ‘Other’

If no federal connection, please complete section 3 and sign and date the bottom of the form.
