Information and Emergency Care Forms

The Health Information Form (HIF) (SS/SE -71) Health Information Form and the Emergency Care Information form (SS/SE-3) Emergency Care Information provide valuable information regarding student health conditions. These forms are critical in the early identification of students who may need healthcare support in the educational environment. Along with the development of Healthcare Plans (HCP) and Action Plans, the Public Health Nurses (PHNs) utilize the information contained in the HIF to develop teaching strategies for FCPS staff in order to meet the requirements of Regulation 2104 Health Services-School Health Related Needs.

For SY 22-23, these forms will be required for all students. Parents/guardians may submit these forms either through electronic submission accessed within the weCare@school module; or thorough Back to School packets for those families without ParentVue accounts.

Please contact your child's teacher with questions about these forms.

Student Rights & Responsibilities (SR&R) Family Signature September 30th

FCPS’ updated Student Rights & Responsibility booklet is now available online and in the ParentVue section of the Student Information System (SIS). The SR&R booklet explains expectations for student behavior and adult responses to enhance school safety and to create a fair, equitable, and supportive school environment.

Each year, parents/guardians must review, sign, and return the SR&R to their child’s school. In ParentVue, you may now download and digitally sign the SR&R. Every parent/guardian with a ParentVUE account will see the SR&R upon login to ParentVUE until the SR&R acknowledgement has been completed.

Your name in the digital signature box must match the parent/guardian’s first and last name in SIS exactly before the signature will be accepted.

Please take a moment to log into ParentVue in your child’s SIS account to access the SR&R. Signatures are due by September 30, 2022.

Please contact the school office if you are having difficulty accessing the booklet.

Parental Consent for Digital Resources

At Sangster, our teachers will use a variety of digital resources to support student learning and promote communication and collaboration with students and families. Some of the digital resources your child may use this year will require your consent before they can be used. You can find our school’s list of the resources that require parental consent here: https://sangsteres.fcps.edu/adopteddigitalresources. Additionally, you can find more information about approved digital resources in FCPS here: https://www.fcps.edu/resources/technology/digital-resources.

Included on this page is also a link to a form where the enrolling parent or guardian will need to indicate whether or not their student is allowed to use the list of digital resources this year. This page also provides more information about how FCPS carefully reviews digital resources for approval and why we must request your consent before using certain tools. We encourage you to complete the parental consent for digital resources form as soon as possible, as your child will not be able to use the digital resources until we have received your permission. Please reach out to Erin Gayzik, Sangster’s School Based Technology Specialist, at emgayzik@fcps.edu if you have questions or concerns about the digital resources we’ve chosen to use this year.

Positivity Project - Week of September 5, 2022

Positivity Project

This week’s character strength is Teamwork. Teamwork is the ability and willingness to place overall group success as a higher priority than individual achievement. Citizenship is a willingness to work towards the common good as opposed to one’s own self-interest. Those with strengths in teamwork and citizenship often work to help all group members find success. These individuals are willing to commit time and resources to further group goals and help move toward the greater good..

The site P2 For Families is available for connecting P2 to your home and you can learn more about the site here. Many of the slides are password protected and the password is P2 (case-sensitive).

Next week’s character strength: Open-mindedness

FCPS Cares!

The FCPS CARES Program is an opportunity for parents, staff, and community members to recognize FCPS employees for going above and beyond to help others and show they care. We invite you to submit your story, so we can all acknowledge the good work of FCPS employees at Sangster ES. We love our Sangster staff members, let's tell them how we feel!
